Add / Amend expiration dates for forms

We issue vendor licenses or temporary licenses that are valid for either 30 or 90 days, vs a full year business license. We need the flexibility to change the expiration date based on the type of license issued. So that it prints on the form for correct expiration. (Form auto fills one year).
